Upstate Respite Services, Inc. (URS) will recruit and train qualified respite workers to provide hourly respite and skill building to human service agencies in the Capital District. Upstate Respite Services will recruit from local colleges and various professional affiliations.  URS will maintain a database that includes profiles of the workers and clients that will be instrumental in identifying closely matched workers for each client. Workers will be matched based on specific variables to include but not limited to geography, disability, age, gender, etc. URS will also maintain an informational website for recruitment and referrals.  URS will be responsible for the interview process, orientation of employees, fingerprinting, required training which includes the writing of progress notes. URS will conduct a criminal background check and a child abuse screening on each employee. Each employee will be responsible for producing a physical exam dated within one year, a valid driver’s license, proof of registration and insurance.  URS will complete an annual satisfaction survey, which will assist agencies in gathering quantitative data in support of their programs. URS will also solicit businesses in the Capital District for pledges of their choice to enhance the quality of respite care. URS employees will be responsible for participating in the initial face-to-face meeting with the family and an agency representative. Following approval of the worker by the family, the worker will schedule their hours according to the family’s needs. URS will provide the worker with a specific format to complete progress notes. URS supervisors will review the progress notes and forward them to subcontractor.
